某院2014~2016年细菌耐药性监测结果分析

摘要(Abstract):

目的探讨某院20142016年细菌分离及耐药性情况。方法收集某院2014年1月2016年细菌分离及耐药性情况。方法收集某院2014年1月2016年12月间临床送检标本,采用微生物常规检测方法分离致病细菌菌株,并对菌株进行药敏实验,分析致病菌分布情况和药敏结果。结果 16 981份标本中,培养分离出细菌3 284株,占19.34%。其中革兰阳性杆菌896株,占27.28%;革兰阴性杆菌2 388株,占72.72%。前5位分离细菌为:大肠埃希菌1 168例,占35.57%;肺炎克雷伯菌466株,占14.19%;铜绿假单胞菌431株,占13.12%;金黄色葡萄球菌388株,占11.81%;鲍曼不动杆菌352株,占10.72%。大肠埃希菌和肺炎克雷伯菌对头孢类、青霉素类、喹诺酮类药物耐药性较高;铜绿假单胞菌对头孢类、喹诺酮类、氨基糖苷类、青霉素类、碳青霉烯类等均有一定敏感性,但鲍曼不动杆菌仅对阿米卡星有敏感性;金黄色葡萄球菌对苯唑西林、红霉素、克林霉素、青霉素G、四环素耐药性较高。结论医院应对致病细菌种类和耐药性进行追踪和分次,针对感染细菌种类和耐药性,指导临床合理使用,以降低耐药菌发生率,提高临床合理用药。
OBJECTIVE To analyze the distribution and drug resistance characteristics of clinical separation germ in a hospital from 2014 to 2016. METHODS Clinical laboratory specimens from January 2014 to December 2016 in the hospital were selected. The microbial susceptibility of isolated strains was detected using the conventional methods, and the drug sensitivity was analyzed.RESULTS Among 16 981 specimens,3 284 bacterial strains were isolated and cultured,of which 896 strains were gram-positive bacteria( 27.28%),and 2 388 strains were gram-negative bacteria( 72.72%). The pathogenic bacteria at the top 5 were Escherichia coli( 1 168 strains,35. 57%),Klebsiella pneumoniae( 466 strains,14. 19%),Pseudomonas aeruginosa( 431 strains,13. 12%),Staphylococcus aureus( 388 strains,11. 81%),Acinetobacter baumannii( 352 strains,10. 72%). Escherichia coli and Klebsiella pneumoniae were more resistant to cephalosporins,penicillins and quinolones. Pseudomonas aeruginosa had certain sensitivity to cephalosporins,quinolones,aminoglycosides,penicillins,carbapenems,but Acinetobacter baumanni is only sensitive to amikacin.Staphylococcus aureus were more resistant to oxacillin,erythromycin,clindamycin,penicillin G and tetracycline. CONCLUSION The hospital should strengthen the monitoring of bacterial resistance and track the results in a timely manner so as to provide reference for the rational drug use in clinical practice.
