新冠肺炎疫情期间标本热灭活后对他克莫司血药浓度的影响

摘要(Abstract):

目的研究标本热灭活前后对他克莫司(FK506)血药浓度是否有影响。方法收集我院最近两周器官移植患者FK506血药浓度标本共计80例,采用化学发光微粒子免疫分析法(CMIA),60例分别测定热灭活前后FK506血药浓度,20例分别测定溶血前后FK506血药浓度。结果 60例热灭活前后FK506血药浓度中,热灭活前平均血药浓度为(6.72±3.58)ng·mL~(-1),热灭活后平均血药浓度为(6.45±3.43)ng·mL~(-1),热灭活前后FK506血药浓度具有显著性差异(P<0.01,P=1.03×10~(-9)),热灭活后血药浓度普遍降低,其中降低占48例(80.00%),差值在-0.8 ng·mL~(-1)至+0.3 ng·mL~(-1)之间。溶血前后FK506血药浓度未见明显差异(P>0.05,P=0.175)。结论热灭活后FK506血药浓度普遍降低,但热灭活后平均血药浓度仅比热灭活前略低。因此,新冠肺炎疫情期间,建议对标本热灭活处理后再进行常规检测,对于保护医护工作者、减少职业暴露和节约紧缺的医疗资源等具有重要意义。
OBJECTIVE To study the effect on the blood concentrations of tacrolimus before and after thermal inactivation. METHODS Collected 80 samples from organ transplant patients to test the blood concentrations of tacrolimus by chemiluminescence microparticle immune assay(CMIA). 60 cases on the blood concentrations of tacrolimus were tested before and after thermal inactivation. 20 cases on the blood concentrations of tacrolimus were tested before and after hemolysis. RESULTS The average of blood concentration in the 60 cases was(6.72±3.58)ng·mL~(-1) before thermal inactivation. The average of blood concentration in the 60 cases was(6.45±3.43)ng·mL~(-1) after thermal inactivation. There was significant difference(P<0.01,P=1.03×10~(-9)) between the blood concentrations of tacrolimus before and after thermal inactivation. The blood concentrations were generally decreased after thermal inactivation with 48 cases(80.00%). the change of difference were between-0.8 ng·mL~(-1) to +0.3 ng·mL~(-1). There was no significant difference(P>0.05,P=0.175) between the blood concentration of tacrolimus before and after hemolysis. CONCLUSION The blood concentrations are generally decreased after thermal inactivation,but the average of blood concentrations after thermal inactivation are slightly lower than before thermal inactivation. So we suggest to test the blood concentrations of tacrolimus after thermal inactivation during COVID-19. It is good to protect the health care of workers,reduce the occupational exposure and save the scarce medical resources.
