儿童哮喘临床药学服务模式的建立及疗效评估Establishment of Clinical Pharmacy Service Mode and Therapeutic Evaluation for Children Asthma

摘要(Abstract):
目的制定临床药学服务模式,探讨其对儿童哮喘临床疗效及治疗费用的影响。方法选取91例学龄期哮喘儿童,随机分为试验组和对照组。试验组在常规治疗外给予临床药学服务,内容包括建立药历、制定哮喘行动计划、用药教育、生活指导以及心理疏导等5个方面;对照组只接受常规治疗。试验组患者毎月随访1次,共随访1年,对照组由医师按常规随访1年。统计干预前和干预后6个月、12个月患儿的肺功能指标(FEV1%、PEF%)、儿童哮喘控制测试(C-CAT)评分及干预前后1年哮喘儿童全年用于哮喘的治疗费用以及临床药师建议被临床医师的采纳率。结果干预后6个月、12个月,与对照组相比,试验组FEV1%、PEF%和C-CAT评分均显著提高,差异有统计学意义(P<0.05);与对照组相比,干预后试验组全年用于哮喘的治疗费用显著降低,差异有统计学意义(P<0.05);而同干预前相比,干预后临床药师建议被儿科医师的采纳率显著提高,差异有统计学意义(P<0.05)。结论该临床药学服务模式可有效改善哮喘患儿的肺功能和哮喘控制水平,降低哮喘治疗费用,值得广泛推广。
OBJECTIVE To develop and implement clinical pharmacy service mode,and to explore the influence of clinical effect and treatment cost for children asthma. METHODS 91 School-age asthma children were randomly divided into experimental group and control group.The experimental groups were given clinical pharmacy service and the routine treatment,including the establishment of drug record,asthma action plan,medication education,life guidance and psychological counseling. The control groups only received routine treatment.The experimental groups were followed up for one times a month,followed up one year,and the control groups were followed up by physicians according routine plays,also one year.The indexes of lung function( FEV1%、PEF%) and asthma control test( C-ACT) scores were measured before intervention amd 6,12 months after intervention.The treatment costs of asthma and adoption rate of clinical pharmacists' suggestion by clinicians before and after intervention were investigated. RESULTS Compared with the control group,the FEV1%,PEF% and C-CAT scores of experimental group were significantly increased( P < 0. 05) in 6,12 months after the intervention,and the asthma treatment costs of experimental group in one year decreased significantly after the intervention( P < 0. 05).Compared with before the intervention,the adoption rate of clinical pharmacists' suggestion by pediatrician were significantly increased after the intervention( P < 0. 05). The differences of all datas were statistically significant. CONCLUSION This clinical pharmacy service model can effectively improve the level of pulmonary function,and reduce the asthma treatment costs.
