摘要(Abstract)：

目的探讨低分子肝素钙在不良孕产史患者解冻移植周期中的应用。方法以某院生殖中心2018年10月～2020年10月收治的行体外受精-卵胞质内单精子注射(IVF-ICSI)周期助孕治疗合并不良孕产史的不孕不育患者131例为研究对象,按照患者就诊顺序编号,采用随机数字表法将其分为观察组(低分子肝素钙联合常规黄体支持,n=65)和对照组(常规黄体支持,n=66),比较两组移植当日的子宫内膜厚度及子宫内膜类型,采用超声检测并比较两组患者移植当日的子宫动脉血流阻力指数(RI),比较两组患者妊娠结局。结果两组移植日各指标组间差异无统计学意义(P>0.05)。观察组生化妊娠率、临床妊娠率、胚胎种植率高于对照组,组间差异显著(P<0.05)。结论低分子肝素钙在不良孕产史IVF-ICSI女性妊娠结局的改善方面具有积极意义,但其具体机制有待更加深入的研究开展。
OBJECTIVE To explore the application of low molecular weight heparin calcium in thawing transplantation cycle in patients with poor pregnancy and parturient history. METHODS 131 infertile patients with adverse pregnancy history treated by in vitro fertilization and intracytoplasmic sperm injection(IVF-ICSI) cycle in a reproductive center from October 2018 to October 2020 were randomly divided into observation group(low molecular weight heparin calcium combined with routine luteal support, n=65) and control group(routine luteal support, n=66) according to the order in which patients wer seen.The endometrial thickness and type of endometrium on the day of transplantation were compared between the two groups.The uterine artery blood flow resistance index(RI) of the two groups on the day of transplantation was detected and compared by ultrasound, and the pregnancy outcome of the two groups was compared. RESULTS There was no significant difference between the two groups on the day of transplantation(P>0.05).The biochemical pregnancy rate, clinical pregnancy rate and embryo implantation rate in the observation group were significantly higher than those in the control group(P<0.05). CONCLUSION Low molecular weight heparin calcium plays a positive role in improving the pregnancy outcome of IVF-ICSI women with poor pregnancy history, but its specific mechanism needs to be further studied.
