一例高血压合并高尿酸患者的药学监护Pharmadceutical Care for A Patient with Hypertension and Hyperuricemia

摘要(Abstract)：
目的探讨临床药师在内分泌科开展药学服务的方法和思路。方法对临床药师在内分泌科参与的1例高血压合并高尿酸住院患者的药学监护过程进行分析总结。结果临床药师通过药物相互作用、药物不良反应和患者用药教育等方面,积极配合临床医师,优化药物治疗方案,提高了患者用药依从性,保障患者用药的安全性。结论临床药师参与临床药学实践,可提高患者用药安全性和有效性。
OBJECTIVE To discuss the clinical pharmacist 's role on pharmaceutical care in endocrinology department.METHODS The clinical pharmacist's effect on pharmaceutical care to a patient with hypertension and hyperuricemia was analyzed and summarized. RESULTS The clinical pharmacist cooperated with the clinical physicians actively through drug interactions and adverse drug reactions and patient education,to optimize the drug treatment and increase patient compliance and enhance the safety of drug application. CONCLUSION Clinical pharmacist can improve the safety and efficacy of patient's medication by participating climical therapeutic practice.
