ALK基因在乳腺浸润性导管癌中的表达及其临床意义

摘要(Abstract):

目的在乳腺浸润性导管癌患者肿瘤组织样本中检测ALK基因的表达状态并研究其临床意义。方法利用荧光原位杂交法(FISH)检测ALK在乳腺浸润性导管患者肿瘤组织样本中的表达,并分析其表达水平与临床特征及乳腺浸润性导管癌患者预后的相关性。结果 FISH结果显示,在所检测肿瘤组织样本中无ALK基因重排阳性病例,但有13例样本存在ALK基因拷贝数增加的现象。此外,ALK基因拷贝数增加与患者年龄(P=0.022)相关,尤其与早期患者预后差(P=0.032)相关。单变量和多变量Cox回归分析表明,ALK基因拷贝数增加是乳腺浸润性导管癌患者的独立预后预测指标(P=0.008)。结论在乳腺浸润性导管癌患者肿瘤样本中ALK基因重排阳性病例少见。但存在ALK基因拷贝数增加的现象,且可能是影响乳腺浸润性导管癌患者预后的不良因素。
OBJECTIVE To examine the status and clinical significance of ALK gene alterations in tumor tissues of breast invasive ductal carcinoma( BIDC) patients. METHODS We detected ALK gene expression in BIDC tissues by fluorescent in situ hybridization( FISH),and assessed the correlation of ALK gene expression with clinicopathologic characteristics and the prognosis in BIDC. RESULTS ALK gene rearrangement positive case was not observed in any BIDC cases in the present study by FISH. ALK gene copy number gain( ALK/CNG) was detected for 13 of BIDC patients. In addition,ALK/CNG-positive in BIDC patients was positively correlated with age( P = 0.022),especially for patients with early stage Ⅰ/Ⅱ( log-rank,P = 0.032). Univariate and multivariate Cox proportional hazards regression analysis suggested that ALK/CNG prevalence was an independent prognostic indicator for patients with BIDC( P = 0.008). CONCLUSION In tumor tissues of BIDC,ALK gene rearrangement positive case is rare,ALK/CNG in BIDC and may be an unfavorable prognostic predictor.
