摘要(Abstract):

目的建立适用于联合用药多的患者万古霉素血药浓度监测的HPLC-DAD法。方法采用高氯酸蛋白沉淀法对血样进行预处理。色谱条件:Agilent 5 TC-C18(2)色谱柱(4.6 mm×150 mm,4.6μm);流动相:磷酸二氢钾(0.05 mol·L-1)-乙腈(92∶8);流速:0.6 m L·min-1;检测波长:200,236,283 nm;柱温:40℃;进样量:20μL。并对60例重症感染患者的万古霉素血药浓度进行检测和分析。结果万古霉素在2.85~91.20μg·m L-1范围内与峰面积呈良好线性关系,最小定量下限浓度:1.0μg·m L-1,日内、日间RSD均<6.7%。60例患者3组波长的血药浓度差异有统计意义,P=0.01(P<0.05)。结论该方法可用于重症感染患者万古霉素的血药浓度检测,并为临床评估疗效、调整给药剂量提供方法学基础。
OBJECTIVE To establish the determination method of concentration of vancomycin serum in patients with multiple drugs by HPLC-DAD. METHODS Plasma samples were precipitated with perchloric acid.Chromatographic separation was performed on a Agilent 5 TC-C18( 2) column( 4. 6 mm × 150 mm,4. 6 μm) with mobile phase consisted of potassium dihydrogen phosphate( 0.05 mol·L-1)-acetonitrile( 92 ∶8) at the flow rate of 0.6 m L·min-1. The wavelength for detection were set at 200,236 and 283 nm.And the column temperature was 40 ℃ with the sample size was 20 μL.The concentrations of vancomycin in 60 severely infected patients was tested and analyzed. RESULTS Vancomycin had shown good linear relationship with peak area at the range of 2.85-91.20 μg·m L-1 at three wavelengths.The limits of quantification was 1.0 μg·m L-1.The RSD values of intra-day and inter-day were lower than 6.7%.The results of three wavelengths had statistically significant differences with P = 0.01( less than 0.05). CONCLUSION The method is sensitive,specific,accurate and reliable,which can be used for therapeutic drug monitoring of vancomycin in severely infected patients.And it also provides a methodological basis for clinical evaluation of therapeutic effects and adjustment of dosage.
